Appealing to Cognitive vs. Emotional Empathy

Thoroughly enjoyed your book. I'm pretty sure you hit on the right conclusions in the closing chapters. I used to work with batterers and some violent offenders. Emotional self regulation and empathy tend to range along a spectrum. The highly emotional men (empaths - as you refer to them) that I worked with responded well to empathy. Men on the sociopathic end of the spectrum tended to view emotional displays of empathy as pathetic and useless. The sociopaths I encountered tended to be quite adept at "cognitive empathy" (the ability to model and predict behaviors from an intellectual perspective), but were blind to feeling. As a consequence, they didn't seem to perceive compassion or guilt. It took me a while to come to the conclusion that the soft, tender, vulnerable part of them just wasn't there in the same way as it often is in others. 

They were however exquisitely stunned to consequences. I learned very quickly to drop empathy as a psychotherapeutic intervention, and to focus on rewards, punishments and outcomes in their lives. They were quick to pick up the fact that kindness was often a far better long term strategy for getting what they want than cruelty. 

Pro social behaviors can be taught to children without a conscience. Parents and teachers just need to know what they are working with. I could go on and describe the manipulative games the sociopaths I worked with used to engage in, the special interest they took in manipulating their therapists, the telltale language they used to describe others, or how they game the system, but you already know these things. I liked working with them. I think I was a puzzle to them. Encountering someone who could be "touchy/feely", and who could abruptly turn off their empathy to confront them directly seemed to confuse them. I imagine that I was able to promote some interest and a sense of unease in knowing that I could see through them. Interesting people. Everyone's trying to make their way in the world.

Different lives, different incentives

This is largely unrelated, except perhaps to the extent that it suggests that normal people often suffer from a lack or failure of not just affective empathy for those people that are considered different from them or are considered too remote to somehow trigger an empathetic response, but also cognitive empathy. People apparently have a very difficult time imagining what the lives of other people must be like.

From the Washington Post, "The big problem with one of the most popular assumptions about the poor", a discussion first of the infamous marshmallow experiment, in which child participants were asked to forego the instant gratification of one marshmallow in order to earn an additional marshmallow. The study participants were followed into adulthood and there was a correlation between ability to wait for the second marshmallow and general success, as society typically defines success. Also correlated, poverty and eating the first marshmallow without waiting. But why? Are they poor because they're impulsive? Or are they impulsive because they are poor? From the article:

The realization has sparked concerns that poverty begets a certain level of impulsiveness, and that that tendency to act in the moment, on a whim, without fully considering the consequences, makes it all the more difficult for poor children to succeed. But there's an important thing this discussion seems to miss. Poor kids may simply not want to delay gratification. Put another way, their decisions may not reflect the sort of impulsive nature we tend to attribute them to.

"When resources are low and scarce, the rational decision is to take the immediate benefit and to discount the future gain," said Melissa Sturge-Apple, a professor of psychology at the University of Rochester who studies child development. "When children are faced with economic uncertainty, impoverished conditions, not knowing when the next meal is, etc. — they may be better off if they take what is in front of them."

In some ways, this uncovers a broader problem with how we perceive the actions of people who live very different lives than we do. We brand certain actions and choices as mistakes, when they might simply be developmental adjustments necessary to cope with their environment. For those who don't worry about their next meal, because they never had to, choosing a marshmallow now instead of two marshmallows in a few minutes, all things equal, could only be the result of impulse-driven folly. For those who do have to worry about the next meal, passing up food now for the promise of food later is the misguided move.

Sociopathic decisionmaking

Along the same lines as yesterday's post, another reader writes about what motivates him (gratification) and what keeps him from doing other things (negative consequences). From a reader:

dear m.e. thomas:

i just wanted to write this to you to express gratitude.  until reading an excerpt from your book, i had always had the vague sense that there was something different about me.  perhaps it was always wishful thinking, or some kind of desire to be special.  who knows? i've never been fully examined by a psychologist or any other kind of -ist, for that matter.  so, after all these years of wondering and suspecting, i came across your writing.

now, all i have is relief.  can sociopaths feel relief?  i don't know.  i have no better explanation for it than that. i am relieved at having read your writing.  the feeling of relief was so palpable that it brought tears to my eyes.

i am 30 years old, and i've felt this way to one degree or another for most of my life. i think it started when i was younger, and had to learn to read people to gauge their responses to my words and actions.  it wasn't until later that i learned that empathy is usually what people use to do those things.  i've always had the burning curiosity, and the desire to experiment.  i truly enjoy experimenting, and over the years it has gotten me into minor troubles, but thankfully i learned early on that i can't just do things for the sake of my own desire.  

i can relate to many things you write about.  i especially relate to the desire to hurt others who have seemingly slighted me.  the only reason i don't act upon my urges is the knowledge of reprisal.  i don't necessarily fear consequence; i simply acknowledge it as being more inconvenient than some short-lived gratification. as a matter of fact, the inconvenience of consequences is the only thing that holds me back from my desires.  the wants themselves run the gamut of importance... sleeping with a woman who isn't my wife is not ethically or socially objectionable to me.  overall, the impact on the world because of 'cheating' is incredibly minimal.  the risk-analysis of temporary physical enjoyment Vs long-term stability is more effective in decision making than any kind of ethics. refusing to slow down at an intersection, when i have the right-of-way and someone pulls out in front of me, is not ethically or socially objectionable to me.  however, going to jail and being locked in a cage seems especially repugnant- not to mention the hassle of repairing my vehicle.

i think things that don't seem to be commonly thought.  i've gone to the point of isolating myself from society because i know i'm different.  the quality of the difference has always been irrelevant, but now i have more to think about.  i constantly want to test and experiment with the 'norm'.  i want to change things, both for the better and for the worse, and i want to observe the reaction.  i WANT to do so much. the gross inconvenience of consequence is the only thing stopping me.  guilt and shame are non-existent. the only thing i truly feel with any kind of passion is a certain amount of hatefulness towards that which i can't control.  the barrier between my wants and satisfaction is maddening.  obviously i can cope with that; you'll not see me in jail any time soon.

please realize that these are merely statements of fact. i would no more act upon them than i would t-bone the car that pulls out in front of me, or fuck a woman simply because i can.

thank you for your writing, and for being straightforward in your words.  if you wish to reply, please do.  if not, i won't really be that upset, will i?

I still get a lot of emails from people that suggest that they don't really understand the sociopathic decisionmaking process. Is it because they make Decision X a particular way, let's say by using empathy, so they expect that anyone without empathy would not be able to reach Decision X? I find that religious people can be this way -- assume that atheists must do bad things because atheists have no reason to be good?  Likewise, do some people believe that sociopaths only choose bad? And if so, what consequences, if any, do they suffer? I feel like there are people who fall into either extreme: believe sociopaths magically get away with things (i.e. won't suffer any consequences) or think that sociopaths will eventually suffer for every choice they make (i.e. karma's a bitch). The reality is that sociopaths probably get caught a little less often than other people because they're better liars and manipulators, but even the best laid Ponzi scheme will eventually collapse. Not all sociopaths are intelligent and sociopaths as a group tend to fail to learn from their experiences (possibly because punishments don't affect them as much as normal people?) but sociopaths are also sensitive to consequences in the form of incentives. So yes, sociopaths are capable of and often do take into consideration consequences (e.g. the reader's comment "thankfully i learned early on that i can't just do things for the sake of my own desire"). And maybe this explains why sociopaths can often function very well for a stretch of time, but willpower has its limits for everyone and sociopaths don't have great judgment, which maybe explains how sociopaths can also self destruct in huge ways.

Or am I wrong? Are sociopaths always scheming ne'er-do-wells? And if so, do they always get what they have coming to them or get away with everything?

Child sociopath: Early intervention as treatment

Sociopathic children represent a unique quandary for people who love children and hate sociopaths. They are also the most impressionable, which some see as an invitation to target them with attempts at early intervention. From Fox News:
Criminologist Nathalie Fontaine of Indiana University studies the tendency toward being callous and unemotional (CU) in children between 7 and 12 years old. Children with these traits have been shown to have a higher risk of becoming psychopaths as adults.

"We're not suggesting that some children are psychopaths, but CU traits can be used to identify a subgroup of children who are at risk," Fontaine said.

Yet her research showed that these traits aren't fixed, and can change in children as they grow. So if psychologists identify children with these risk factors early on, it may not be too late.

"We can still help them," Fontaine said. "We can implement intervention to support and help children and their families, and we should."

Neuroscientists' understanding of the plasticity, or flexibility, of the brain called neurogenesis supports the idea that many of these brain differences are not fixed.

"Brain research is showing us that neurogenesis can occur even into adulthood," said psychologist Patricia Brennan of Emory University in Atlanta. "Biology isn’t destiny. There are many, many places you can intervene along that developmental pathway to change what's happening in these children."
How do you accomplish this early intervention?
"You don’t have to do direct brain surgery to change the way the brain functions," Brennan said. "You can do social interventions to change that."

Fontaine's studies, for example, suggest that kids who display callous and unemotional traits don't respond as well to traditional parenting and punishment methods such as time-outs. Instead of punishing bad behavior, programs that emphasize rewarding good behavior with positive reinforcement seem to work better.

Raine and his colleagues are also testing whether children who take supplemental pills of omega-3 fatty acids — also known as fish oil — can show improvement. Because this nutrient is thought to be used in cell growth, neuroscientists suspect it can help brain cells grow larger, increase the size of axons (the part of neurons that conducts electrical impulses), and regulate brain cell function.

"We are brain scanning children before and after treatment with omega-3," Raine said. "We are studying kids to see if it can reduce aggressive behavior and improve impaired brain areas. It's a biological treatment, but it's a relatively benign treatment that most people would accept."
I'm conflicted about this news. On the one hand I am reminded of very well-meaning attempts at early intervention for children of first generation immigrants, Romani children, aboriginal children, native children, and other efforts at forced integration and assimilation into the "norm." On the other hand some sociopaths really do suffer immensely. To the extent that these efforts demonstrate society's increasing awareness of sociopaths and desire to cater more carefully to sociopathic needs, e.g. using very consistent incentive schemes instead of punishment to achieve desired behavior, I see it as a step in the right direction.

Sociopath treatment

There is no "cure" for sociopathy, but it can be managed well or it can be managed poorly. Sociopaths don't respond very well to punishment, but they do respond to incentives. As discussed in this little blurb, referencing the successful treatment of teenage sociopaths:
Psychopaths aren’t murderers and criminals by their nature, they’re simply people without compunction. Naturally this can make them callous and cold, but it doesn’t automatically make them dangerous. In fact, in cases where it has been demonstrated and proven to psychopaths that they will personally benefit from abiding by rules, then it is possible they can become relatively well assimilated into society. Let me explain, where traditional treatment is used, (like psychotherapy or counseling), the sessions merely act as a kind of unintentional specialized training to help psychopaths more effectively mimic healthy emotional behavior. They are not treated or helped by traditional therapy as any insane (or sane) person might be. But Dr. Robert Hare developed a system whereby by accepting that the psychopath may never be anything but rational and uncaring, and used logical process to demonstrate that it is in their own interest to behave appropriately. Apparently there are criminal rehabilitation systems which are testing out this process, offering instant rewards for psychopathic subjects who follow the rules, appealing to their need for instant gratification and it is shown to be relatively effective. This is the first treatment in the history of psychopathy that has been been effective at all.
What is the difference between a successful sociopath and a less successful one? Sometimes I wonder if it's a matter of chancing upon a good thing first and sticking with it rather than a bad one -- like we're monkeys in an experiment, and if you press one button you get heroin, if you press another you get food, and if you press another you somehow get sexual pleasure. Once you find a good thing, you'll probably just stick with it, right? I always got good results being legit, ever since I was little, but if I didn't, or if my first good results came from being less legit, I'm sure I would have gone that way.

But if you want to "manage" yourself or someone else, I do think the key is incentives and immediate gratification. The caveats are that the incentive cannot be discretionary (because sociopaths will see it as arbitrary and not actually associate it with their own behavior) and the reward can't seem too good or the task too hard, otherwise he will spend all of his time trying to figure out how to trick you into giving him the reward without actually completing the task.

Nature vs. nurture

This New York Times article states the obvious -- bad parents can't take all the credit for good children and good parents can't take all the blame for bad children. Interestingly, it goes out of its way to say that bad behavior does not necessarily equal sociopathy:
“I don’t know what I’ve done wrong,” the patient told me.
She was an intelligent and articulate woman in her early 40s who came to see me for depression and anxiety. In discussing the stresses she faced, it was clear that her teenage son had been front and center for many years.
When he was growing up, she explained, he fought frequently with other children, had few close friends, and had a reputation for being mean. She always hoped he would change, but now that he was almost 17, she had a sinking feeling.
I asked her what she meant by mean. “I hate to admit it, but he is unkind and unsympathetic to people,” she said, as I recall. He was rude and defiant at home, and often verbally abusive to family members.
Along the way, she had him evaluated by many child psychiatrists, with several extensive neuropsychological tests. The results were always the same: he tested in the intellectually superior range, with no evidence of any learning disability or mental illness. Naturally, she wondered if she and her husband were somehow remiss as parents.
Here, it seems, they did not fare as well as their son under psychiatric scrutiny. One therapist noted that they were not entirely consistent around their son, especially when it came to discipline; she was generally more permissive than her husband. Another therapist suggested that the father was not around enough and hinted that he was not a strong role model for his son.
But there was one small problem with these explanations: this supposedly suboptimal couple had managed to raise two other well-adjusted and perfectly nice boys. How could they have pulled that off if they were such bad parents?
To be sure, they had a fundamentally different relationship with their difficult child. My patient would be the first to admit that she was often angry with him, something she rarely experienced with his brothers.
But that left open a fundamental question: If the young man did not suffer from any demonstrable psychiatric disorder, just what was his problem?
My answer may sound heretical, coming from a psychiatrist. After all, our bent is to see misbehavior as psychopathology that needs treatment; there is no such thing as a bad person, just a sick one.
But maybe this young man was just not a nice person.
For years, mental health professionals were trained to see children as mere products of their environment who were intrinsically good until influenced otherwise; where there is chronic bad behavior, there must be a bad parent behind it.
But while I do not mean to let bad parents off the hook — sadly, there are all too many of them, from malignant to merely apathetic — the fact remains that perfectly decent parents can produce toxic children.
When I say “toxic,” I don’t mean psychopathic. . . .
I often tell readers that not every asshole ex of theirs is a sociopath, and the same applies for misbehaving children. In this situation, though, I actually think it is foolish to discount the potential role of sociopathy. There is a strong genetic but weak environmental link to sociopathy, which is consistent with having two normal sons and one sociopathic one. Furthermore, although inconsistent discipline may not be enough to cause anyone to become a sociopath, it could trigger sociopathy in someone who was genetically predisposed to it, as sociopath children are particularly sensitive to incentive structures and perceived fairness (i.e. consistency and reciprocity). I obviously don't know the full story, but just based on the article, the description fits sociopathy, at least for this kid.

After spending time with my family recently, I am more convinced that nurture had a significant role to play in my development into a sociopath. When people ask me whether I had a bad childhood, I tell them that it was actually relatively unremarkable, however I can see how the antisocial behaviors and mental posturing that now define me were incentivized when I was growing up -- how my independent emotional world was stifled and how understanding and respect for the emotional world of others died away. Still I don't think I was "made" into a sociopath, nor was I born one. I feel like I was born with that predisposition, that I made a relatively conscious decision to rely on those skills instead of developing others, and that the decision was made in direct response to my environment and how I could best survive and even thrive in that environment. It's a bit similar to this author's description of her own survivalist adaptations:
If you’ve read much about writers, you know that many of us grew up with an alcoholic parent or in some otherwise dysfunctional home. Me, too. Kids who are raised in households where feelings of safety and predictability are up for grabs might be more likely to turn into storytellers. We spend a lot of emotional energy trying to guess what might happen next, and mentally drawing up different contingency plans. It puts us in the “what if” habit early.
Genetics are important for sociopathy, but environment plays a crucial role as well. Although the NY Times article notes that "[f]or better or worse, parents have limited power to influence their children," such that they should be reluctant "to take all the blame — or credit — for everything that their children become," unfortunately (or luckily?) they can still take quite a bit of blame (or credit) for sociopathic children, particularly with new "studies suggesting that such antisocial behavior can be modified with parental coaching." Knowledge is power.

Training the low-functioning sociopath

A reader asks how she can better interact with her lower-functioning sociopath ex for the benefit of their child:
Growing up with an extremely high functioning sociopath for a best friend, I perhaps not understand but appreciate the logical thinking that comes along with not having to consider others' emotions. I've seen the game well played, but my soon to be husband keeps shooting himself in the foot. He is now destitute, living without so much as a car and is about to lose everything he will ever make in the divorce settlement. Have you met any sociopaths who completely sabotage themselves to this degree? Is this a challenge to him, an insult to me, or a result of a game gone horribly wrong? Or am I missing the point entirely? Is there a way to encourage him to use his advantages for more productive means or am I dealing with a real chump and should call it a day?
My response:
Sociopaths are allegedly slow learners, particularly experiential learning, which I'm sure is a large part of why sociopathic criminals are very likely to re-offend. Sociopaths don't respond effectively to punishment because they don't fear punishment in the same way that neurotypicals do. Dr. Hare illustrated this in a study where participants watched a timer ticking down to a painful electric shock. Normal people would get increasingly anxious as the time for the shock approached, but psychopaths did not. Because sociopaths do not fear consequences in the same way empaths do, they are prone to making the same mistakes over and over again. Even mice can be trained to stop pushing particular buttons that lead to certain negative consequences, but sociopaths struggle. Because sociopaths are risk takers, they also tend to be overly optimistic about their chances. Finally, sociopaths are largely emotionally empty -- their lives do not have much meaning beyond the power they can acquire and gratification in which they can indulge, so they don't have much to lose.

Still, if sociopaths seem oblivious to punishments, at least the more successful ones are surprisingly sensitive to incentive systems. My personal theory is that the lowest functioning sociopath is only sensitive to immediate rewards, while the highest functioning sociopath has learned to also feel pleasure in accumulating delayed rewards. For example, low functioning sociopaths might be enticed to do an honest day's work for a honest day's pay, but are less likely to save money for retirement. For whatever reason, I am able to experience part of the pleasure today of anticipated rewards, which is probably why I managed to fund my retirement by age 30?

How would i use this knowledge to train a low functioning sociopath? I think the same way that you might train a small child or a pet -- break up rewards into small, frequent, easy to achieve increments. For instance, if he does a small task, you will let him have more time with his daughter, you will let him borrow your car, you will let him be late with support payments, whatever the small reward you want to provide is. He probably won't like thinking that you are messing with him or manipulating him, which to him will smack of a power struggle or paternalism, so try to find some natural connection between his actions and your responses to his behavior. For example, if he picks up your daughter on time, you won't have to pay the babysitter extra money to stay, so you are willing to send her out with spending money. Or his calling ahead when he cannot make his appointed visitation allows you to make other arrangements so you do not need to miss work. In economic-speak, sociopaths are rational actors, perhaps the purest form of rational actor, as that term is defined. You can always count on them to look out for their best interest, and as long as what you want is also something they want, they can act in your best interest as well.

Mellowing with age?

I've written a little about sociopaths mellowing with age. I was reminded of the concept again when I read this recent comment:


I've reached the point where I'd like to get some help for my impulsive traits. I'm' saying "impulsive" rather than "psychopathic" because I'd like to focus on what I see as the problem.

You could say that I'm a high-functioning psychopath. I'm Machiavellian, narcissistic and psychopathic. 

I've apparently got enough impulse control (and intelligence) to get me what I need. But I sense that there's a gap. I'm missing something.

In the past, I wouldn't have done this because I was too proud. I wasn't able to admit that I was behaving "badly" or had a problem. And I was young enough that I was substantially getting what I wanted. 

Now that I'm middle aged, that isn't happening. Perhaps because I'm not getting what I want, I can see I've behaved anti-socially, and it has cost me. E.g. I'm heading into old age, and no matter how optimistic I'd like to be, it is clear that it is going to get harder and harder to have sex with women in their twenties. And eventually I will die, no matter how much I fight it.

Have any commenters, motivated by the same sense of "oh shit I'm screwed" sought help? How'd it go? 

I do have this general sense of trending towards being more actively aware of potential consequences of my actions to the point where I am able to assert more "self-control" than I could as a young person. And sometimes people say that I seem like I've gotten better -- either people in my life or even sometimes commenters on the blog in response to things I have written. But's also interesting to re read other posts like this one, where as recently as two years ago I wanted to burn it all to the ground. But I didn't. Maybe that's the difference that's come with age.

Educating children sociopaths

This was an interesting article about the delicacy between not wanting to diagnose sociopathy in children because it can be such a damning prognosis, but also wanting to do it to possibly put at-risk children in some sort of early intervention program. As researcher put it:

To me it seems a no-brainer,” says Essi Viding, professor of psychology at University College London. “Nobody’s going to get psychopathy as a present when they turn 18. Of course you’re going to see some precursors."

What might early intervention look like?

“If you’re labelling someone a psychopath, it does seem to assume that there’s nothing we can do for them, that they’re going to grow up to be a criminal, and that you might as well just lock them up,” Frick says. “But you can teach a child to recognise the effects of their behaviour.”

The lack of success in educating children sociopaths to recognise the effects of their behaviour may have more to do with using the wrong methods than it does a sociopath child's inability or unwillingness to learn:


“What [people who use social shaming as a method of punishment] assume is that children have the motivation to shift their behaviour. That their primary social motivator is the relationship,” says Warren.

She interviewed more than 1,000 children with behavioural problems, aged eight to 18. “I was beginning to see children who weren’t responsive to these interventions, who weren’t interested in what adults, parents, educators, think,” she says.

“If a child or group of children isn’t interested in pleasing someone, doesn’t care about sanctions, what they’re driven by is pleasure or reward. They will take the pain of the consequences but won’t change their behaviour.”

What does work?


The programme, called Let’s Get Smart, replaces sanctions with rewards. Some teachers were uneasy. “Some teachers felt that punishment happens in the real world; if they misbehave in the real world they will still go to prison. Why are we setting them up for unrealistic expectations?” says Jones. “Our point is that it doesn’t work.”

By offering regular rewards, perhaps three times a day, controlled by the adult in authority, it aims to provide a rational, self-interested motivation for pleasing adults where that motive is emotionally absent. “The adult becomes the clear intermediary between the child and what the child wants,” Warren says. The rewards are tailored to each child’s interests.

These children often have a strong desire for control and teachers have to resist attempts to negotiate, because any concession just leads to more demands. “I teach parents and teachers to say, ‘It’s not open for discussion, go away.’ Adults don’t like to dismiss children,” Warren says.

All this is backed by role play and other exercises that are intended to build children’s capacity to pay attention to and respond appropriately to others’ emotions. Video playback helps the children to see their behaviour as others do, often to their surprise. (“I do swear a lot,” one girl told Warren.)

What sort of results can you expect?

“These kids are still having quite significant problems: they are not cured. But they are improving. Some have gone back to mainstream school, which is incredible,” she says.

Whether children ever really learn greater empathy or just learn to manage their behaviour is not clear. “My personal gut feeling is that you can modify behaviour perhaps more readily than you can improve the empathy response,” Viding says.



There are a couple of other interesting tidbits, like this one about "circle time" type emotional sharing activities:

“(In circle time) children say what makes them scared and children who have callous-unemotional traits think, ‘This is a useful piece of information to have’,” she says.
